Application Procedure for Certificate of Entry (COE)

for medical treatment in Thailand

While Thailand still puts in place travel restriction, non-Thai nationals who wish to seek an medical treatment in Thailand can apply for the Certificate of Entry. They can travel to Thailand with up to 3 accompanying persons.

There are necessary steps to be taken as follows:

1. Applicant must contact the Alternative Hospital Quarantine (AHQ) for Confirmation letter with the stamp and Affidavit of Support Letter with the stamp (if you have accompanying persons). Please check the example of certificate here

***The Alternative Hospital Quarantine (AHQ) will convey your application to the Ministry of Foreign Affairs Thailand to approve your COE and inform directly to the embassy***

2. If you received the certificate from your AHQ, the following documents are required for application:

2.1 fill in the declaration form (download from here) and submit it to the Embassy via email [email protected] at least 15 working days before intended date of travel. 

2.2 Copy of valid passport

2.3 Health insurance with a minimum coverage of USD 100,000 for medical treatment expenses of COVID-19 during stay in Thailand. The health insurance scheme must indicate that it covers illness from COVID-19.

2.4 Copy of valid  Visa (if you have)

2.5 A confirmation letter with the stamp 

2.6 Affidavit of Support Letter with the stamp (if you have accompanying persons)

2.7 Financial evidence that you have sufficient fund to pay for your medical treatment in Thailand, such as bank statement
 
2.8 Proof of relationship with the accompanying person(s) (if have accompanying persons with a copy of passports)

**** The Embassy may ask for more documents as deemed necessary ****

3. After the embassy receives the approval of your COE from the Ministry of Foreign Affairs Thailand, applicant need to inform Royal Thai Embassy, Helsinki if you would like to travel with the repatriation flight (arranged by Thai Government). The Embassy will reserve a repatriation flight upon the passengers’ intended date of departure. Once the embassy receives the confirmation of the flight, the passengers will be informed of the details of the flight and how to reserve ticket accordingly. Then the passengers are required to reserve the ASQ and submit its confirmation of ASQ and flight ticket to the Embassy to proceed with the Certificate of Entry (COE).

the Embassy will inform the applicant to collect the Certificate of Entry (COE)/ submit a visa application (if need) which will be issued subject to the verification of the additional documents in item 3. The appointment for collection will be sent via email.
 

Please note that the Embassy cannot confirm how long it will take and whether it will be approved or not.

4. To collect the Certificate of Entry (COE), the applicant must present the additional documents and their copies to the consular officer as follows:

4.1 Passport

4.2 Confirmed air ticket (No need if you travel with the repatriation flight (arranged by Thai Government))

4.3 An original COVID-free Health Certificate (COVID -19 is not detected and the COVID test must be by RT-PCR method), issued no more than 72 hrs. 

4.4 An original Fit-to-Fly certificate from a doctor, issued no more than 72 hrs. (This certificate must be seperated from Covid-free Helth certificate)

4.5 A confirmation letter of the Alternative Hospital Quarantine (AHQ)

*** The applicant must arrange their own air tickets and a 14-day Alternative Hospital Quarantine (AHQ) reservation at his/her own expenses. More information, including the list of AHQ in Thailand can be found at http://hsscovid.com/.

5. The applicant who does not have a valid  Visa,  you will get an appointment to submit a visa application after your COE is approved.

6. During flight check-in for departure, the applicant must present the airline officer the followings:

6.1  Certificate of Entry, issued by the Royal Thai Embassy.

6.2  Declaration form

6.3  An original COVID-free Health Certificate (COVID -19 is not detected and the COVID test must be by RT-PCR method), issued no more than 72 hrs. 

             6.4 An original Fit-to-Fly certificate from a doctor, issued no more than 72 hrs. (This certificate must be seperated from Covid-free Helth certificate)

6.5  Health insurance with a minimum coverage of USD 100,000 for medical treatment expenses of COVID-19 during stay in Thailand.

7. Upon entry to Thailand, Traveler entering Thailand during the COVID-19 pandemic period must submit a T.8 form upon his/her arrival in the Kingdom of Thailand. (Download T.8 form here or register online through AOT phone application at https://aot-app.kdlab.ai/ )

8.  Please note that 14 days before traveling to Thailand, traveler shall avoid being in crowded area or the area which is risky to get infected by COVID-19.
